  • Call for RT/help.
  • 100% oxygen; disconnect/bag if needed.
  • Confirm tube with waveform EtCO₂.
  • Check DOPES: displacement, obstruction, pneumothorax/patient, equipment, stacked breaths.
  • Treat hypoxia/hypotension in parallel.
